Admissions

Our Professional Team will offer insight, direction, and assistance with the admission and referral process. Our goal is for both parties to identify if our program would be a great fit for the child. We would like to see every child succeed.

The Process

Provide Documentation

The referring person/agency should provide:

psychological evaluations, court reports, case management evaluations, any medical information, and documentation of social or problematic information to consider. The Admission Team will review the packet.

Interview

A phone and/or in-person interview will be scheduled with the child. The interview provides opportunity for the child to identify their current challenges, ask questions, and express what they feel their treatment needs may be. The child, their family, and a person from the referring agency are welcome to join in on the interview.

Acceptance

After review of the child’s information, a determination of acceptance will be provided within 24-48 hours. If a bed is not available at the time of acceptance, the child will be added to our waiting list.

Placement

Once placement of the child has been determined, an intake date shall be set. At intake the child and parent/legal guardian will meet with a team member to begin filling out paperwork. The team member will answer any questions regarding the program and the services we provide to the residents. The intake procedure is designed to welcome the child and parent/legal guardian, along with providing additional program information about Tree of Life Arizona.
